The Director of Case Management holds a crucial role within the hospital setting, overseeing the implementation and operation of the case management program. Their primary responsibilities encompass several key areas, including:

  • The director is responsible for ensuring that the case management program is effectively established and operational within the hospital.

  • The director ensures that discharge planning processes are in place to facilitate safe and timely transitions for patients leaving the hospital, including coordinating post-discharge care and support services.

Overall, the Director of Case Management plays a critical role in optimizing patient care delivery, resource utilization, and care coordination within the hospital environment.Primary Responsibilities:

  • Provide leadership, education, and supervision for Case Managers and Social Workers, ensuring the smooth day-to-day workflow of the department.
  • Monitor the Case Management Department’s documentation to ensure it meets regulatory compliance standards.
  • Maintain proficiency in case management and utilization review to provide coverage for patient caseloads and address staffing needs across all hospital areas.
  • Communicate with physicians regarding patient needs, assist in developing appropriate treatment plans, and facilitate level of care and bed placement assignments.
  • Directly responsible for personnel actions, including hiring, conducting performance appraisals, creating employee schedules, and maintaining payroll records and time reports in KRONOS.
  • Facilitate daily Multidisciplinary Rounds to foster collaboration with other disciplines and ensure holistic patient care.
  • Participate in discharge planning, providing necessary education and resources to meet the discharge needs of individual patients and families.
  • Actively participate in Utilization Review Committee and Revenue Cycle Committee meetings.
  • Promote efficient utilization of clinical resources and ensure appropriate resource allocation based on patient acuity.
  • Ensure understanding, awareness, and compliance with all applicable Joint Commission, CMS, state, and local agency laws, regulations, guidelines, policies, procedures, and professional standards.
  • Perform other duties as assigned by hospital administration.Knowledge, Skills & Abilities:
  • Comprehensive knowledge of Medicare, managed care systems, and the entire healthcare continuum including inpatient, outpatient, and home health services. Familiarity with utilization management, discharge planning, and case management processes.
  • Ability to collaborate effectively with healthcare professionals across all levels to achieve established goals and enhance quality outcomes. Strong interpersonal skills facilitate productive teamwork and interdisciplinary cooperation.
  • Working familiarity with performance improvement concepts and methodologies, allowing for the identification of areas for enhancement and the implementation of strategies to drive continuous quality improvement.
  • Self-motivated with demonstrated assertiveness and excellent communication skills. Capable of working autonomously while also thriving as an integral team member, facilitating effective communication and cooperation among colleagues.
  • Proven track record of establishing and maintaining effective working relationships with physicians, fostering mutual respect and collaboration to ensure the delivery of high-quality patient care.Experience Requirement:Candidates must have at least two years of hands-on experience in Case Management, specifically in utilization management, case management, discharge planning, cost/quality management programs.Candidates with two to three years of prior management/Supervisory experience, including a minimum of two years in hospital-based nursing roles.Certificate/License:Holders must possess Iowa Mandatory Reporter certificates for Child and Dependent Adult Abuse.Possession of a current RN license issued by the state of Iowa, or a multistate license permitting practice within Iowa, is required.
