We are seeking a new Licensed Clinical Social Worker to join our team. Your role will be to Provide psychotherapeutic services and crisis intervention for pediatric patients. Conduct screenings and assessments for behavioral health conditions using standardized screening tools (PHQ-9, GAD-7, SBIRT). Collaborate with medical staff to integrate behavioral health care with primary health care leading to better patient outcomes. Participate in multidisciplinary teams to manage chronic health conditions and develop preventative health strategies. Maintain detailed and accurate patient records in the Electronic Health Records (EHR) system. Engage in community outreach activities and health education initiatives. Provide culturally sensitive care adapted to the needs of diverse populations.. As a member of the Primary care department, you will work with multi-disciplinary team and report to our Practice Administrator.
