Infusion & Home Health Care Registered Nurse (RN)

Reports To: Medical Director

Job Summary:

Infusion and Home Health Care RN will apply all nursing skills in this role, including infusion, wound care, ostomy teaching, medication teaching, and disease management. Infusion and Home Health Care RN will deliver care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. Infusion and Home Health Care RN will collaborate with other medical team members to ensure effective patient care delivery and achievement of desired patient outcomes. Nurses effectively interact with patient, while maintaining the standard of professional nursing.

Job Duties and Specifications:

• Assess patients and documents pertinent information, completing nursing assessments.
• Based on the assessment of new patients, create a plan of care in conjunction with physician orders
• Performs medication administration as per MAR protocol. Accurately record all medication as administered.
• Assesses, documents, and reports behavioral and medical changes Participates in patient treatment planning as part of the multi-disciplinary clinical team.
• Provide hands-on nursing skills along with patient/family education per the patient's plan of care.
• Educates patients on diagnosis, treatment, and medications.
• Can describe and demonstrate thorough knowledge of JC standards and guidelines, as required.
• Records all care provided concisely, accurately, and completely, in a timely manner, as per the agency protocol
• Draw labs and provide wound care as ordered.
• Coordinates with all providers to ensure patient medical and psychiatric needs are met such as scheduling outside appointments and ensuring exchange of information
• Requests patient information such as charts from previous providers
• Other duties as assigned

Minimum Skill Qualification:

• Maintain an understanding of state and federal regulations, JC Standards and facility licensing requirements.
• Understand client/patient/resident rights.
• Provide treatment that promotes client dignity, independence, individuality, strengths, privacy, and choice.
• Be able to recognize and respect cultural differences.
• Ability to speak Arabic a huge plus
• Works well with minimal supervision
• Demonstrate ethical behavior, such as by respecting staff member and client boundaries and recognizing the inappropriateness of receiving gratuities from a client.

Minimum Qualifications:

• Must be graduate of an accredited Registered Nurse Program - preferably BSN
• AZ Level 1 Fingerprint card (required)
• You have reliable transportation with auto insurance and active drivers license.
• Negative drug screen and tuberculosis (TB) test within the last 12 months.
• Valid CPR and First Aid Certificate.
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds.
• Adhere to HIPAA and maintain client confidentiality.
• Possess a valid and in good standing RN license in the state of Arizona
• 2 years’ experience working with patients in home health.

Job Types: Full-time, Part-time, Contract, PRN

Pay: $35.00 - $40.00 per hour
