Arizona Life Hospice – Mohave is looking for a Hospice Nurse Practitioner to join our NP rotation by providing on-call services to our clinicians Monday through Sunday (7 days on 7 days off) and to perform home/telehealth face to face visits. Face to Face schedule will be provided 30 days in advance. 

The primary objective of the Nurse Practitioner is to assist in oversight of the clinical care and treatment of patients served by Arizona Life Hospice - Mohave, and to consult and cooperate with the patients’ attending physician, if any, and in collaboration with the Hospice Medical Director. The Nurse Practitioner works in conjunction with the Hospice Medical Director and any other members of the delivery team to:

·Serves as an extension to the patient’s attending physician ensuring that the plan of care is implemented as successfully as possible to promote the patient’s comfort and safety in their environment.

·Provide direct medical care as needed to patients, ensuring collaboration and coordination with the attending physician, and other providers, in the care of the patient seeking opportunities for improvement both in terms of outcomes and efficiencies.

·Assists in developing the plan of care for each patient/caregiver and assures the quality of comprehensive plans of care with other members of the care delivery.

·Reviews the quality of symptom management and modifies plan as needed in collaboration with the attending physician, patient, family and members of the care delivery team.

·Communicates effectively, timely, and appropriately at all times.

·Assures the accuracy and timeliness of documentation in compliance with CPT codes, face to face documentation and clinical notes.

·Ensure patient care is delivered at the appropriate level of care with coordination of PCP and medical director, provide management of medication profile to prevent polypharmacy and ensure optimal symptom management

·Utilize the medication formulary whenever possible for the management of patient symptoms

·Practice a high level of ethics and professional standards and be responsible for knowing and adhering to Company policies and procedures.

·Follows and adheres to all federal, state and accrediting nursing standards and practices

·Be capable of using Electronic Medical Records.

·Is a member of and participates in designated interdisciplinary group activities.

·Provides advice, guidance, and assistance to Hospice staff until a satisfactory resolution is reached when a medical order:

-Is of a questionable nature.

-Contains a discrepancy.

-Lacks clarity.

-Continues to be a concern for the staff after consultation with the primary physician.

·Provides medical consultation and direction when the attending physician or their designee cannot be reached and there is a change in the patient’s condition requiring medical attention.
