ResponsibilitiesThe Therapist provides patient care including evaluating the patient for treatment providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management while delivering quality patient services.
