AristaCare Health Services is Hiring a Social Worker (FT) Near Manchester, NJ
AristaCare at Manchester is looking for a Sub Acute Social Worker!
As a Sub Acute Social Worker, you will assist with planning, developing, organizing, implementing, and evaluating the social service programs of this facility.
Responsibilities include but not limited to:
- Participate in community planning related to the interests of the facility as well as the services and needs of the resident and their family.
- Participate in discharge planning, development and implementation of care plans and resident assessments.
- Involve the resident/family in planning social service programs when possible.
- Assist in arranging transportation to other facilities when necessary.
- Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
- Must be able to participate in room moves.
Requirements:
- An individual with a minimum of a Bachelor’s Degree in Social Work or a Master's degree in Social Work
- Must have LCSW or CSW
- Long term care experience preferred
- Excellent interpersonal and conflict resolutions skills.
