Responsibilities
· Provide comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and seniors.
· Provide health coaching and education to patients on various topics such as nutrition, exercise, and disease prevention.
· Coordinate and manage the care of patients with chronic conditions, including discharge planning and follow-up appointments.
· Documents all patient encounters.
Qualifications
· Master’s degree in nursing from an accredited program.
· Successful completion of a Family Nurse Practitioner program is a requirement.
· National Certification as a Family Nurse Practitioner is necessary.
· Must be licensed to practice in the District of Columbia.
· Current CPR certification (ALS or BLS for the Healthcare Provider or equivalent) is mandatory.
· Strong verbal and written communication skills are essential.
· Ability to thrive in a team-oriented environment is crucial.
· Demonstrated sensitivity to ethnic minority communities and a commitment to serving underserved populations.
· Preferably, one year of professional experience in ambulatory health.
· New-to-practice candidates with a solid medical nursing background are encouraged to apply.
· Proficiency in computer skills, with prior experience using an electronic medical record system preferred.
· Fluency in Spanish and/or French, is an asset.
