Field Registered Nurse, Home Health
Hourly Rate: up to $45/hr
Minimum Qualifications:
1. Colorado RN license
2. 6 months clinical experience, but will consider new Grads!
3. Reliable transportation
Level of Responsibility:
Works under the supervision of the Director of Clinical/Professional Services or designee.
Exercises supervision of assigned personnel including licensed practical nurses, home
health aides, personal care providers and office support staff.
Examples of Duties and Responsibilities:
1. Initial and ongoing comprehensive assessments including Outcome and Assessment
Information Set (OASIS)
2. Initiates the plan of care and revises as necessary
3. Teaches, supervises, and counsels the client and family members regarding the disease
process, self-care techniques, prevention strategies, and other related problems of the
client. Promotes client/family independence
4. Provides those services requiring substantial and specialized nursing skill.
5. Initiates appropriate preventive and rehabilitative nursing procedures.
6. Supervises and demonstrates appropriate procedures as necessary to LPN’s and aides.
7. Maintains adequate records and reports including those required by Medicare and other
third-party payers.
8. Informs the physician and other personnel of changes in the client's needs.
9. Evaluates the effectiveness and outcomes of care.
10. Plans for the client's discharge from service.
11. Serves as coordinator of client's care including referral to other services as appropriate
12. Supervises LPN’s and home health aides.
13. Participates in the in-service programs.
14. Performs other related functions as required.
Knowledge, Skills, and Abilities:
1. Thorough knowledge and skill in current nursing practice.
2. Knowledge of the concept of team nursing.
3. Knowledge of the development of plans for client care and the ability to participate in
them.
4. Ability to accept and utilize supervision.
5. Ability to recognize the needs and concerns of people, and to maintain constructive
working relationships in dealing with them.
6. Ability to supervise.
7. Ability to communicate effectively both orally and in writing.
