Community Health Worker - GPSThe Area Office on Aging services a 10-county area in NW Ohio by coordinating and providing local services to older adults, their families, and people with disabilities. Job SummaryThe Community Health Worker (CHW) supports the GPS program, case managers, and participants by helping individuals and their families navigate and access community services and other resources. Essential Job Functions
Responsible for establishing trusting relationships with the community, program participants, and Team members.
Assist community members, program participants, and team members by providing support and education during care transitions and community engagement.
Implement educational programs and empower individuals to increase health knowledge and Healthy behaviors.
Collaborate and perform needs assessments and program evaluations.
Develop and implement care plans based on health assessments.
Identify & address individual and community needs around health education and access to healthcare resources.
Facilitate context-appropriate health education.
Conduct home or phone visits to provide good care coordination, support, and guidance.
Provide community resource referrals, participant advocacy, support, outreach, and follow-up.
QUALIFICATIONSHigh school diploma or equivalent; associate degree in human services, Psychology, or related field preferred. Experience working in a community-based setting for at least 1-2 years is preferred. Must have CHW certification or complete CHW certification within the first six months of hire. Knowledgeable of state agencies or Medicaid approval process. Reliable transportation, insurance, valid driver’s license, the ability to satisfactorily undergo an FBI/BCI check and a Motor Vehicle Record check (MVR) on at least an annual basis.
