Responsible for the bi-monthly collection of health statistics of the clients involved in the Produce for Better Health program through the Agency. Two to four times each month, health statistics including weight and blood pressure will be collected, recorded in a log, and entered in a database for the Nutrition Manager to submit to the State of Indiana. This position will be open from May 1, 2023, through June 30, 2025. Job duties include: Regularly collect health stats from clients at the congregate meal sites in all six counties. Collect weight and blood pressure data from every client involved in the PBH grant program. Track data in a database correctly using excellent entry skills to assure correct and current data is entered without error. Submit the database to the Nutrition Manager no later than the 1st day of each month. Adhere to all Agency policies regarding HIPAA and PHI information. Must have a valid driver’s license, a reliable vehicle, and adequate auto insurance. Must be an RN or LPN. Must have excellent people skills, have an excellent driving record, and meet the minimum requirements for employment with the Agency. Preferred prior experience working with geriatric clients and have knowledge of proper use of blood pressure and weight scale equipment to ensure proper and uniform data collection. This person will be standing for long periods, lifting up to 20 pounds, and must be able to work at least 8 hours per day. Each congregate meal site offers a positive client-centered atmosphere that must be respected during any interaction with the Agency’s clients.
