Position is required to:

• Conduct quarterly visits to Veterans home.
• Complete new assessments within three days of referral to intake
• Complete Veterans intake of new referrals including submission of Veterans Directed Home Care enrollment paperwork
• Assist with prioritization of consumers for program and service eligibility by completing 701S assessment in accordance with DOEA program standards
• Build a relationship with Veteran clients to ensure veterans have a complete understanding of pertinent policy information and program requirements regarding available care options
• Provide accurate explanation to veterans about all programs and services available to Veterans including veterans with short term urgent needs.
• Work with consumers over the phone to elicit needed information in order to properly assess needs and make eligibility determination or to make referrals to appropriate community resources.
• Meet with new enrolled veterans to properly assess needs, values and circumstances in order coordinate appropriate services
• Ensure veteran receives appropriate oral and written communication and follow up regarding approval of paperwork and any changes in status
• Communicate with contractor regarding enrollments, budget plan revisions, and other Veteran/Fiscal Agent related matters
• Assist Veterans with timekeeping and process semi-monthly payroll

Minimum Requirements for position:

• GED or HS Diploma and a minimum of three (3) years’ experience in case management, social services, call center services or related work experience OR
• AA degree in a human service filed and a minimum of one year of experience in information and referral services, case management, social services, call center services or related work experience OR
• Bachelor’s Degree in a human services field
• Additional language of Spanish or Creole a plus

Partial remote schedule offered after successful completion of training.
