THIS IS AN OVERNIGHT SHIFT POSITION. WORK HOURS ARE 10:00 P.M.-6:00 A.M.

Provides assistance in daily living and personal care in patients' homes in accordance with an established care plan.

• Follows assigned plan of care and notifies staff nurse of changing or unusual conditions.
• Assists patients by providing personal services such as grooming, bathing and dressing.
• Supports patients by providing housekeeping and laundry services; shopping for food and other household items; preparing and serving snacks and assisting with eating; as defined in assigned plan of care.
• Assists client with mobility/transfer, getting in and out of bed or chair and/or walking.
• Obtains emergency assistance if required.
• Participates in all scheduled in-service meetings.
• Projects a professional image and attitude at all times. Wears company-issued ID badge and company-approved uniform while on duty.
• Provides pet care services for patient such as dog walking, taking dog on bathroom breaks and cleaning up/disposing of waste properly, emptying cat litter boxes, feeding pet, etc.
• Provides companion care such as retrieving and reviewing mail with patient, sitting with patient and reading aloud, sitting and conversing with patient during dining times, and walk the grounds with patient.
• Acts as escort to/from dining hall and other activities on the property.
• Properly utilize applications as required such as Paylocity, Authenticare, MatrixCare, HHAExchange, and Carebridge.
• Other duties as assigned.

Requirements

• Minimum of a High School Diploma or equivalent.
• Must possess a valid driver’s license.
• Reliable transportation and proof of state required vehicle insurance.
• Current negative TB skin test or state health card.
• Completion of 75 hour Home Health Aide training class.
• Certified in CPR/First Aid.
• Smart phone with acceptable internet capabilities and data limits required to perform certain functions of position.
