Overview:

Responsibilities

* Conducts enrollment assessments to determine clinical eligibility.

* Completes UAS - NY Assessment instrument.

* Maintains frequent communication with the Home Care Department and Care Management to keep ArchCare Senior Life a primary option for care delivery.

* Identifies any barriers to implementation of Plan of Care.

* Communicates with the Care Team in conveying information to facilitate any concerns, or answer questions.

* Completes UAS - New York Assessment instrument as assigned to determine continuing eligibility and / or identify any needed changes to Care Plan.

* Performs other related duties, as required.

Job Type: Per Diem

Salary: Fee-for-service structure. $225 per assessment, $250 per new enrollment

Qualifications

* Registered Nurse, currently licensed and registered to practice in New York State.

* UAS-NY Certified

* Bachelor's Degree in Nursing preferred.

* Experience in Home Care, case management, or long-term home care.

* Demonstrated skills in planning, organizing and directing the work of others required.

* Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.

* Proficient with DOH UAS assessment instrument.
