Job Overview:
We are seeking a highly skilled and experienced Memory Care Clinical Director to join our team. As the Clinical Director, you will be responsible for overseeing the operations and clinical services of our organization. You will provide leadership and guidance to our team of healthcare professionals, ensuring the highest quality of care for our residents.

POSITION SUMMARY

Assumes primary responsibility for coordinating the Care services on a 24 hour/7-day week basis for residents residing in Open Arms Neighborhood. Participates in creating Functional Needs Assessments/Level of Care, Service Planning, as well as renewal and maintenance of these plans. Conduct assessments whether on site or off to determine appropriate placement. Recommends resident service goals and assists in their implementation. Works with the Director of Resident Care and Wellness to supervise, oversee and coordinate the work of Resident Care Specialists to meet resident needs. Provides oversight of resident needs and ensures all training of Resident Care Specialists is completed in a thorough and timely manner. Works with Director of Resident Care and Wellness/Executive Director to ensure all regulatory requirements are in place and a comprehensive safety plan is adhered to. This position works a minimum of 2 eight-hour shifts on the medication cart weekly. Works closely with residents and their families to ensure their care needs are addressed in accordance with the mission, established policies, procedures, and guidelines of Arcadia Senior Living.

ESSENTIAL FUNCTIONS

  • Manages the services of all Memory Care residents, including the dissemination of information to families and staff.
  • Performs on-site assessments prior to move in.
  • Performs on-site assessments of residents admitted to hospitals and/or SNF and maintains contact with resident’s families with the intent of returning resident to Arcadia Senior Living.
  • Completes assessments prior to move in, 30 days after move-in, every 6 months and or any significant change as well as per state regulations.
  • Ensures all required documentation is completed prior to or upon resident admission, including nursing assessment and PPOC as applicable per regulations.
  • Observes residents and records significant conditions and reactions, as well as notifying physician of resident’s condition and reaction to drugs, treatments, and significant incidents. As applicable per state regulations.
  • Facilitates continuity of care for those residents receiving home health care services.
  • Develops, organizes, and conducts service plan reviews with resident families and pertinent care team members.
  • Ensures in house ancillary medical services, such as podiatrist, doctor visits, Psych visits, lab, x-ray, ambulance, etc. are scheduled and followed through.
  • Responds to life saving situations based on nursing standards, policies, and protocol.
  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home‑like environment for all Residents.
  • Adheres to all established policies and procedures.
  • Responds and addresses Resident or staff concerns as necessary.
  • Maintains appropriate resident records and ensures these records are kept current and accurate.
  • Assists in maintaining good public relations, participates in marketing, activities and assists in recruiting staff.
  • Monitors quality of work performance of Resident Care Specialists and reports to The Director of Resident Care and Wellness.
  • Assures infection control procedures are known/followed by staff.
  • Attends care plan conferences, maintains service/care plans, task sheets and functional needs assessments for each Resident as required within ALINE.
  • Is on‑call according to pre‑established schedule, refers for emergency care, as appropriate.
  • Observes all work, safety, and administrative rules.

21. Acts as Resident advocate in medical‑social situations involving Resident and/or designated agents as allowed by Personal Care/Assisted Living Licensure Requirements.

  • Addresses physical, social, spiritual, and psychological needs of Residents.
  • Coordinates move‑out of Residents with The Director of Resident Care and Wellness/Executive Director, as assigned.
  • Reads staff communications and maintains familiarity with Resident records as required.
  • Assures proper records are maintained.
  • Keeps all information confidential.
  • Maintains acceptable attendance records/follows work schedule.
  • Organizes and utilizes time appropriately toward accomplishing assigned tasks and requires minimum supervision.
  • Displays cleanliness, good grooming, personal appearance, and follows dress code requirements consistently.
  • Ability to work safely and to recognize potentially dangerous situations.
  • Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security, work, safety, and other emergency procedures.
  • Attend scheduled Management Meetings and participate as Weekend Manager on Duty as assigned.
  • Identifies Resident and staff training needs.
  • Conducts training as assigned by Executive Director in coordination of The Director of Resident Care and Wellness and mandated by Licensure requirements.
  • Orients and instructs Resident Care Specialists in coordination of The Director of Resident Care and Wellness.

  • Designs a creative and exciting life enrichment program to meet the individual needs and interests of the residents residing in the Open Arms Neighborhood
  • Ensures and instructs the Memory Care Programming Coordinator the ongoing social, spiritual, educational, and recreational activities for residents of all levels, abilities, interests, and ages.
  • Ensures and instructs the Memory Care Programming Coordinator that a daily, weekly, and monthly calendar of events based on the residents’ interest is in place. The calendar should include individual and group activities, social and recreational events, internal and outside activities, physical, education and spiritual opportunities, entertainment, etc.
  • Evaluate and develop new activities for residents with various interests.
  • Maintain direct communication with staff and families regarding activities, special needs, and common area set-up and availability.
  • Complete the “Getting to Know You” and “Event Interest Questionnaire” forms with new residents and as required by state regulations.
  • Tastefully Decorates the Neighborhood for the season, holiday and or event.
  • Work within a monthly budget. Retain records of past activities.

SUPERVISORY RESPONSIBILITIES

Directly supervises the Resident Care Specialists, Memory Care Programming Coordinator in the Open Arms Neighborhood and based upon the staffing model of the community. The director is responsible for the overall direction, coordination, and evaluation of staff or volunteers in the Open Arms Memory Care Department. Director carries out supervisory responsibilities in accordance with Arcadia policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

The Open Arms Director also serves as back- up for The Director of Resident Care and Wellness with both working together in the overall operation of the Care Department. May serve as backup for LPN on the medication cart (if applicable) and resident assistants as needed.

QUALIFICATION STANDARDS

  • Must possess a current license as LPN or RN.
  • Ability to be friendly, positive, professional, and cheerful; interact easily with residents, resident families, 3rd party care providers, visitors, and co-workers.
  • Ability to work in stressful situations, remaining calm and demonstrating good judgment and self-control in emergency and difficult situations.

· Reaching heights of approximately 6 feet and depth of 2 ½ -3 feet

· Must be able to work irregular hours under pressure and stress.

· Must have high level of mobility/flexibility in space provided.

· Ability to do occasional heavy lifting (up to 50 lbs), 30 lbs. regularly.
