Aptive is Hiring a Primary Care Nurse Practitioner Near Fayetteville, NC

Job Summary

Aptive Resources, in collaboration with the Fayetteville VA Medical Center, is seeking a highly skilled and compassionate Primary Care Nurse Practitioner to provide comprehensive family practice consultative services to Veterans in an outpatient setting. As a member of the Primary Care team, you will play a vital role in assessing, diagnosing, and managing the healthcare needs of our nation's heroes.

Schedule: Monday through Friday 8am-5pm

Primary Responsibilities

  • Conduct thorough patient assessments, including health history, physical examination, and diagnostic evaluations.
  • Provide patient education on disease prevention, treatment options, and medication management.
  • Order and interpret diagnostic tests as necessary, and collaborate with physicians to develop appropriate treatment plans or referrals.
  • Record and document all patient encounters accurately and in a timely manner using the electronic comprehensive patient record system (CPRS).
  • Develop and implement individualized care plans based on patient needs and preferences.
  • Monitor patient progress, adjust treatment plans as needed, and ensure continuity of care.
  • Collaborate with interdisciplinary team members to coordinate care and facilitate communication between primary care providers and specialty services.
  • Adhere to VA policies, procedures, and standards of care to ensure compliance and quality of service delivery.

Minimum Qualifications

  • Master's degree in Nursing from an accredited program.
  • Current, unencumbered license to practice as a Nurse Practitioner in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • United States citizenship.
  • Certification in VHA-approved Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) from the American Heart Association.
  • Certification as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC).
  • Minimum of 5 years of experience as a Nurse Practitioner in family practice within the last five years.
