Care Coordinator: FT 37.5 hours per week, benefit eligible. Schedule determined by program need. The Care Coordinator is responsible for coordinating, assessing, and scheduling all admissions for the program. They are the primary contact for all referrals to the program and will be the primary point of contact for other providers, referral sources, and potential clients. The Care Coordinator will promote engagement and retention in treatment and facilitate an effective discharge plan whereby the individual successfully connects to the next level of care. The Care Coordinator will work with a multidisciplinary treatment team to help individuals engaged in the Residential Program to achieve their substance use recovery goals. This position may include weekend and evening hours.
