Population Health Analyst
We are seeking a dedicated and experienced Population Health Analyst to join our team and play a key role in improving healthcare outcomes across multiple provider groups. The Population Health Analyst will be responsible for leveraging data insights to support the relationship between the provider practice and apree, implementing a detailed opportunity analysis, identifying opportunities for improvement, supporting reporting and presentation to provider practices. The ideal candidate will possess strong analytical skills, relationship management, and a passion for driving positive change in healthcare delivery.
Responsibilities:
●Implement comprehensive opportunity analysis to identify areas for improvement in population health management within multiple provider groups.
●Running reports from payor and population health platform to derive insights
●Provides and presents practice performance, financial overviews and economic impact related to performance and contract terms to relevant practice stakeholders, clinical and administrative.
●Complete end of year supplemental data transmission to payor
●Communicate outcomes-oriented overviews of performance initiatives to convey organizational impact.
●Develop and nurture lasting relationships and engagement with practice care teams to support and enhance financial and quality performance across performance metrics.
●Serve as a resource and subject matter expert on population health management, sharing knowledge and best practices with providers, staff, and other stakeholders.
●Develop and implement strategies, in partnership with their clinical counterpart, to optimize clinical workflows and care coordination processes to support population health initiatives.
●Collaborate with interdisciplinary teams to develop and implement population health programs and interventions aimed at improving health outcomes and reducing disparities.
●Monitor and evaluate the effectiveness of population health interventions and initiatives, making adjustments as necessary to achieve desired outcomes.
Qualifications:
●Bachelor's degree in related field; Master's degree preferred.
●Registered Nurse (RN) strongly preferred
●Strong healthcare knowledge and understanding of value-based care contracts across all lines of business (commercial, ACA, MA)
●Strong analytical skills with the ability to make meaning of large data sets and identify opportunities required.
●Strong presentation skills and an ability to communicate at an executive-level
●Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with provider practices.
●Proven ability to prioritize and manage multiple tasks in a fast-paced environment.
●Proficiency in project management
●Demonstrated problem-solving skills and the ability to navigate complex healthcare systems.
●Excellent organizational and project management skills, and team-building skills
●Willingness to travel locally as needed and flexible availability to accommodate patient and provider schedules.
