Applied Home Health Network is Hiring a Home Health Care Supervisor Near Chicago, IL

Job Overview:

As a Home Health Care Supervisor, you will play a crucial role in overseeing the delivery of high-quality care to our patients within their homes. You will be responsible for supervising and leading a team of home health aides and nurses, ensuring that they adhere to established care plans and maintain the highest standards of patient care. The ideal candidate will possess strong leadership skills, clinical expertise, and a commitment to excellence in home healthcare services.

Responsibilities:

1. Supervise and manage a team of home health aides, providing guidance, support, and direction as needed.

2. Conduct regular assessments of patient needs and progress, adjusting care plans as necessary to ensure optimal outcomes.

3. Ensure compliance with all regulatory requirements, agency policies, and procedures related to home healthcare services.

4. Provide ongoing training and education to staff members to enhance their skills and knowledge.

5. Monitor the quality of care provided by staff members through regular supervision, performance evaluations, and quality improvement initiatives.

6. Act as a liaison between patients, caregivers, and healthcare team members to facilitate communication and resolve any issues or concerns.

7. Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.

8. Participate in agency meetings, committees, and other activities to support organizational goals and objectives.

Requirements:

- Previous supervisory or leadership experience preferred.

- Strong communication, interpersonal, and organizational skills.

- Strong computer literacy including proficiency in Microsoft Office Suite

- Exceptional organizational skills with the ability to prioritize tasks effectively

- Ability to multitask and work well under pressure in a fast-paced environment

- Professional appearance and demeanor at all times
