Applied Counseling and Consulting Services is currently seeking a Systematic Skill Builder at our agency for part time employment (~6 hours per week) for multiple consumers within the Wernersville region.

JOB RESPONSIBILITIES:

• Developing methods of tracking effectiveness of behavior support plan.
• Complete Direct care visits and telephone consultations
• Monitoring and analyzing data and make changes when necessary based on data collection.
• Collaborating with the person serviced treatment team, including participation in the person supported ISP meetings
• Ensure clients’ health, safety, dignity, and individual rights at all times while enhancing the quality of the individuals’ everyday life.
• Provide services to each person in their home and/or their community according to their Goals and Objectives.
• Complete and submit all daily progress notes and time sheets in a timely and professional manner, in accordance with Applied Counseling Guidelines.
• Provide transportation for the person to local community outings or scheduled appointments as needed.
• Utilize local community resources in order to promote community integration, independence, and interdependence.

REQUIREMENTS:

• Bachelors Degree in Social Work, Psychology or Education preferred
• Excellent communication and computer skills
• Valid driver’s license

Job Type: Part Time - Must be willing to travel designated area of coverage.
