Appleton Area Health is Hiring a MDS Coordinator Near Appleton, MN

POSITION

The MDS Care Coordinator will oversee the entire operations of the RAI/Care Plan/MDS process. The MDS coordinator will be available for flexible hours as needed to complete all assessments in a timely fashion. The MDS Care Coordinator should have a general knowledge of the floor nursing process to cover the nursing floor in an emergency as well as be able to supervise/direct floor nursing processes efficiently and effectively. This position is supervised by the Director of Nursing.

STANDARD REQUIREMENTS

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Knowledge of case mix, Medicare, and Federal and State LTC regulations.
  • Ability to work independently and make decisions.
  • Possess verbal, written, and organizational skills.
  • Knowledge of gerontology and rehabilitative nursing principles and professionals.
  • Maintain membership in professional organizations and participate in meetings and conferences.

MINIMUM TRAINING AND EXPERIENCE REQUIREMENTS

  • Graduate of an accredited school of nursing and licensed as a Registered Nurse in the State of Minnesota, bachelor’s or graduate degree in nursing or administration desirable.
  • Minimum of two years of direct experience in geriatric nursing.
  • Supervisory experience preferred.

ESSENTIAL FUNCTIONS

  • Always demonstrate AAH Values.
  • Communication with Residents, families, visitors, and coworkers using AIDET & Key Word principles. Listen attentively & observe both verbal & non-verbal communication.
  • Participates in facility/departmental strategic planning and decision-making and keeps current and implements new practices as needed.
  • Participates in quality initiatives and risk management by providing input and ensuring improvement in quality of care and safety for residents and staff; and strives for ways to improve overall activities and functions of the care center and the facility.
  • Protect and promote residents’ rights and choices.
  • Maintain standards of regulatory agencies (CMS, MDH, OSHA, OHFC) and submit required reports.
  • Be available to residents and families as a liaison and follow through in an appropriate manner and to promote good public relations with residents, families, visitors, volunteers, and the community.
  • Works collaboratively with others to accomplish goals/tasks and maintain interdepartmental relations to ensure quality and continuity of care.
  • Coordinates, develops, monitors, and maintains current resident assessments as required.
  • MDS-Initial, quarterly, annual, and significant change assessments.
  • Resident care plans - assures that initial temporary care plan is initiated upon admission, writes Master Care Plan upon completion of MDS, prepares quarterly summaries, and attends Care Conferences. Keeps Short Term Care Plans current.
  • Develops, implements, and initiates all other assessments that are necessary.
  • Initiate and follow PPS assessments/RAI/MDS/Care Plan schedule.
  • Coordinates RAI/PPS activities with other departments. Responsible for timely submissions.
  • Communicates effectively with Interdisciplinary Team members.
  • Consults with DON on nursing problems and interpretation of nursing home policies to ensure resident needs are met.
  • Engages in studies and investigations related to improving nursing care.
  • Will have a broad knowledge of nursing home rules and regulations.
  • Attends mandatory in-services offered by the facility.
  • Assures resident safety.
  • Attends facility Medicare meetings.
  • Must possess strong leadership qualities and the ability to direct staff in the best interest of resident quality of care.
  • Must be able to manage time efficiently and work independently to complete all tasks in a timely manner.
