Summary of Essential Duties:
To advance ethical practice and professionalism while supporting protection emotional stability and healthy childhood development through service coordination, linkage, advocacy, etc.
Compile all relevant information and documentation pertaining to youths
Participate in service coordination on behalf of youth and families, under supervision of the MDT and/or licensed therapist
Work in tandem with the licensed therapist, youth, family/guardian and all other members of the MDT providing care coordination input in the development of individual service plans, treatment plans and IMDS tools
Serve as School, Community, and Ancillary liaison.
Represent the youth and family on the agency's behalf as it relates to screening, placement, court appearances, etc.
Remain in consistent contact with all members of the youths MDT (internal/in house and external)
Provide referral and linkage to ancillary and or specialized treatment services and interventions
Routinely conduct school, home, and ancillary visits
Schedule and attend special appointments (medical, allied, etc.)
Responsible for correspondence on behalf of the youth and family-treatment team meeting invitations, as well as monthly/quarterly plan dissemination to members of the MDT
Participate in treatment conferences, individual educational plan, child family team, emergency review meeting etc.
Actively participants in, MDT, House Rounds, Clinical Rounds and Record Reviews
Responsible for PerformCare authorization status and ensuring Medicaid coverage eligibility
Qualifications:
BSW or BA, BS, in Human Services, Psychology, Sociology or related fields of study
Valid NJ/PA Driver's License.
At least three years of experience working with at risk youth.
Candidate must pass:Criminal Record History, FBI and Child Abuse History Clearance
