We are actively recruiting a compassionate Registered Nurse Case Manager to join our dedicated team, focusing on the Inland Empire and High Desert areas. In this role, you'll experience a hybrid position blending hands-on patient care with essential administrative case management tasks.

Responsibilities:

• Establish and maintain relationships with patients, families, and caregivers
• Conduct patient visits following the care plan, assess needs, and document as per Medicare COPs
• Communicate patient conditions to the office and supervisory staff
• Complete necessary documentation accurately and in a timely manner
• Participate in on-call and after-hour rotations with full-time employment

Requirements:

• Current RN License
• Valid Driver's License and Auto Insurance
• Recent Physical and TB test
• Hospice experience preferred
