Apex Community Care is Hiring a Program Quality & Reporting Coordinator Near Danbury, CT

Position Summary:

Oversight of the agencies Quality Management (QM) program including Ryan White, Prevention, Behavioral Health and Medical.

Responsibilities:

  • Coordinate Apex compliance issues assuring quality plans are met
  • Prepare monthly reports on program status and progress and other reporting as needed
  • Manage, develop and implement standards of care
  • Coordinate closely with Apex CEO and program Directors
  • Manage the monitoring, measurement, and review of internal processes, especially those that affect the quality of the organization’s services
  • Attend community meetings when requested
  • Conduct periodic management review meetings
  • Assist in training new employees in regards to data requirements
  • Facilitate monthly QM meeting with funders
  • Present data to funders and provide suggestions on improving service measures that are not meeting required goals

Qualifications:

  • Bachelors Degree Preferred
  • Knowledge of program services
  • Attention to detail and proficiency in communication
  • Strong organizational skills
  • Ability to work closely with others as a part of a team
  • Internet media and computer proficiency
  • Great communication skills

Pay: $25.00 - $31.00 per hour

Expected hours: 37.5 per week
