Summary
Under the direct supervision of the Clinical Program Manager, the Case Manager IV (ICMS) will be responsible for providing Intensive Case Management Services (ICMS) for people who were formerly homeless and who have health disparities with a focus on Behavioral Health access. The Case Manager IV will be responsible for monitoring client activities in a residential housing program to ensure compliance with program policies and expectations The Case Manager IV will provide culturally appropriate/competent support programs for individual involved with all components of behavioral health and psychosocial support. Experience working with diverse ethnic and cultural communities is a must, including, but not limited to the following target populations: people of color, gay/lesbian/bisexual/transgender individuals, individuals with limited English proficiency, undocumented immigrants, substance users, and homeless persons, of varying ages.
Essential Functions
1. Conduct client intake screenings into behavioral health and psychosocial support program services, and assessment plans.
2. Be stationed at a permanent residence to provide targeted case management services, including but not limited to linkage and referral to food assistance, childcare, transportation, housing, care coordination, employment development and skills building.
3. Support residents recertify their HACLA application to remain housed on an annual basis.
4. Mediate between resident and landlord to avoid eviction.
5. Conduct group facilitation sessions.
6. Develop and implement outreach, recruitment, and linkage referrals to community-wide stakeholders.
7. Provide necessary program documentation, including the completion of reports, administration of evaluation tools, and other program related assignments.
8. Attend and actively participate in unit, case conference, internal staff in-service, community, and other relevant agency events.
9. Ensure the safety of clients and the security and sanitation of the housing program.
10. Ability to work overtime and perform duties during weekends, holidays, and evenings when required.
11. Maintain and uphold Agency mission statement, values, policies, procedures, and principles.
12. Other activities as required to conduct program and agency objectives.
13. Regular attendance required.
Minimum Qualifications - Knowledge, Skills and Abilities Required
1. High school diploma or GED equivalent and above, or at least 2 years of experience in the HIV/AIDS or behavioral health field.
2. Knowledge of HIV/AIDS, and related issues.
3. Knowledge of and ability to work with racial/ethnic minority, impoverished and underserved communities and groups (e.g., multi-gender, generation, LGBTQ, substance using).
4. Strong interpersonal skills and ability to work with staff from diverse cultural, ethnic, educational, and socio- economic backgrounds.
5. Proficient writing and oral communication skills.
6. Demonstrated experience and knowledge of PC hardware and software (i.e., Windows, word processing, spreadsheets).
7. Ability to coordinate multiple tasks simultaneously in high pressure environment.
8. Maintain and uphold Agency mission statement, values, policies, procedures, and principles.
9. Valid California Driver License, Proof of Car Insurance, and reliable transportation.
10. Verification of employment eligibility and background check.
11. Updated tuberculosis test (annual requirement) – read, reviewed, and dated by a clinician 6 months prior to employment.
Preferred Qualifications - Knowledge, Skills, and Abilities
1. Experience working in a non-profit community-based organization.
2. Experience providing counseling grounded in evidence-based practices such as Seeking Safety or TREM.
Supervisory Responsibilities
1. The incumbent in the position does not have formal supervisory responsibilities.

1. Ergonomically safe office environment with desktop computer, desk, chair, natural light from windows.
Physical Requirements
1. While performing this job, this position typically involves driving, sitting, standing, walking, carrying (max. 20 lbs.), lifting (max. 20 lbs.), listening, and speaking.
