AOM Healthcare is a premier long term care company, operating twenty one long term care facilities throughout the state of Ohio and is now hiring a Full-Time Social Services Director at Medina Center for Rehabilitation & Nursing, located at 555 Springbrook Dr, Medina, Ohio.

Job Duties & Requirements:

• Identifies, Evaluates and documents the medically related social, emotional and personal needs of the residents.
• Coordinates with family, community and staff in meeting these special needs.
• Conduct pre- admission and admission meetings with families and prospective residents.
• Provide crisis intervention for residents and families when appropriate.
• Makes appropriate arrangements for discharge planning
• Provide in-service education to the staff on the social and emotional needs of the residents and families
• Coordinates self-help therapeutic groups to enable the residents and families to promote their own well-being and create a support structure within the facility.
• Maintain a high community profile especially with service providers, referral agencies, the medical community and government agencies.
• Attend staff meetings, department head meetings, resident care conferences and corporate social service designee meetings.
• Performs other duties as assigned.
