Job Duties & Requirements: 

The Admissions/Social Services Designee is responsible for providing tours, internal marketing, screening residents for admission, identifying potential admissions and communicate accurate facility census, in addition to other job duties and responsibilities related to the Admissions, Social Services process.

  • Act as a liaison between families, residents, and employees for complaints, concerns, requests, and information, and assures that care standards are met and the highest degree of quality resident care is provided at all times.
  • Advocate for and educate residents and employees of the Residents' Bill of Rights
  • Cultivate/Maintain professional relationships in the local health care markets
  • Generate referrals for services by building relationships with hospitals, physicians, independent and assisted living facilities and other community resources
  • Coordinates tours, admissions, insurance verifications & pre-certs
  • Facilitates admissions decisions, and ensure positive admission experience for customers and families
  • Extraordinary customer service to both external and internal customers. External customers would include referral sources, patients, and their families. Internal customers would include supervisors, clinical & non-clinical staff members, pharmacies, and suppliers
  • Visits residents and assists with resources to promote well being
  • Investigates complaints initiated by residents, family, staff, or advocacy/regulatory agencies
  • Assists residents with adjustment period
  • Provides and maintains resident advance directives
  • Attends resident care plan meetings and functions as a contributing member
  • Prepares and maintains Social Services documentation
  • Conducts resident groups per specific, identified topics/needs
  • Assists with annual in-service on resident rights
  • Adheres to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
  • Assists with Resident Council
  • Advocates and informs residents and family members of Bill of Rights
  • Makes referrals to physician for dental, podiatry, optometry and psychological services as needed
  • Assists families in knowing about bed hold status
