The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 

Jordan Health is now seeking a Medical Records Correspondence Technician who, under the guidance of the Health Information Management Supervisor, is primarily responsible for providing complete, accurate and timely medical record support services, which includes the “Correspondence Window”.

Requirements

The Medical Records Correspondence Technician will ensure to:

· Complete and responds to all request for medical records, medical related information, and legal matters via telephone, fax, in-person request, etc. 

· Assists providers in the completion of patient forms and preparation of forms, letters, etc. for provider signature. 

· Prepares medical information, via release of information request forms, for contracted vendor to process request. 

· Assist in retrieving medical information for internal and external audits, site surveys and participates in quality assurance projects, as needed, under the supervision of the Director of Health Information/Quality Improvement. 

· Processing of incoming electronic faxes and directs to appropriate staff. 

· Scans and indexes medical documentation correctly in the electronic medical record. 

Education And Experience Required:

· Minimum High School Diploma.

· AAS degree preferred.

· Minimum one to two years of experience in release of information/correspondence required. 

·

Licenses And Certifications Required:

· RHIT Certified Preferred.

Special Skills, Knowledge Required: 

· Proficient computer skills.

· Experience working in electronic medical record (EMR) system preferred.

· Strong organizational skills and ability to multi-task.

· Ability to read and interpret medical records, forms, and authorizations.
