Overview:
We are seeking a dedicated Nurse Practitioner to join our healthcare team. The ideal candidate will have a strong background in various medical specialties and possess exceptional skills in patient care and treatment. The Certified Registered Nurse Practitioner (CRNP) is a member of the Hospice Interdisciplinary Care Team (IDT) who works in collaboration with the hospice Medical Director, physicians and members of the IDT to provide hospice services to patients and their families. The CRPN engages in independent decision-making about health care needs and provides health care services to individuals and their families. The CRNP provides direct patient care as well as physician-based consultative and administrative services to assure state of the art hospice care. The hospice CRNP implements his/her role in a self-directed, motivated, and highly organized manner, while demonstrating excellence in assessment, diagnosis and treatment skills, and verbal and written communication skills that positively impact the hospice customers experience and satisfaction.
Duties:
1. Provides general medical care and assessment patients in medical facility or patients homes. Under direction of physician: performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.
2. Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
3. Records physical findings, and formulates plan and prognosis, based on patient's condition.
4. Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
5. Prescribes or recommends drugs or other forms of treatment.
6. Consults with hospice physician and the IDT for treatment plan/updating plan of care.
Qualifications:
1. Completion of graduate level nurse practitioner education program accredited by a national accrediting body and documentation of certification by a national credentialing body.
2. Knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis, and treatment in a hospice setting.
3. Knowledge of Medicare "Face to Face Encounter" criteria.
4. Ability to work with other health care providers to provide appropriate care.
5. Knowledge of interdisciplinary team concept.
