Requirements
- Valid RN license in the Commonwealth of Pennsylvania
- Clinical experience in Advanced Wound Care, Wound Vacs, ostomies, COPD, CHF, diabetes management, IVs
- Experience coordinating patient care
- Professional and courteous demeanor when speaking with patients
- Excellent communication skills
Clinical Care Coordinator Tasks
- Perform initial home health assessment call with patient/caregivers
- Assess patient for home health appropriateness
- Determine immediate care needs of the patient
- Assist patient in scheduling follow-up appointments with PCP
- Discuss and coordinate care with PCP
- Assist in ordering supplies for patients
- Assist patient in receiving additional covered services
- Perform telehealth visits with patients when needed
- Support Administrator and Clinical Supervisors in daily tasks
- Support Intake Specialist in daily tasks
- Participate in staff meetings
- Complete documentation timely and accurately
- Provides effective communication reports through written, verbal, reporting
- Support field clinicians in coordinating supplies for patients
- Perform chart reviews
- Assist in QAPI activities
- Participates in the on-call rotation
