Position Overview:

Annex Healthcare, a leading home healthcare agency in Southern California, is seeking dedicated and passionate recent LVN graduates to join our team. As an Entry-Level Home Health LVN, you will provide in-home care to pediatric or adult patients with complex needs, including G-Tubes, Trachs, and ventilators. This role offers flexible shifts (8, 10, 12 hours), allowing you to create a schedule that suits your needs and priorities.

Locations: La Crescenta, Granada Hills, Agoura Hills, Canoga Park, Sun Valley, Woodland Hills, A Glendale, Los Angeles, Santa Clarita, San Fernando valley and Lancaster.

Requirements:

• Current CA state license to practice as a Licensed Vocational Nurse.
• Recent graduate from an accredited LVN program.
• Current BLS/CPR Certification.
• Strong desire to provide compassionate care to patients in the comfort of their own homes.
• Ability to work in various conditions and travel to patients' residences.
• Physical ability to stoop, bend, lift, and transfer patients.

Responsibilities:

• Provide in-home care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN Case Manager.
• Administer medication, manage chronic illnesses, and assess changes in patients' medical status.
• Assist patients with daily living activities and maintain their activities of daily living.
• Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
• Collaborate with physicians, other agency staff, and contract services to coordinate and implement the Plan of Care.
• Complete all required documentation in accordance with agency standards and regulatory requirements.
