Annandale is Hiring a Waiver Program Manager & DDP Near Suwanee, GA

Position Description: Coordinate and implement the Community Access Program. Oversee and ensure the implementation of supports and services identified in each Villager’s Individualized Support Plan. Serve as an intermediary for the Villagers, families, other agencies, and support coordinators, by collecting data, goal tracking information, progress notes, schedules, events and activity documentation. Serves as the Developmental Disability Professional (DDP) for the program by following all regulatory requirements for the position. 

Minimum Qualifications:

Bachelor's Degree in Social Work or related human services field and/or 5 years of experience in a human services setting, preferably in MR/MH/DD. Must be able to meet the minimum requirements outlined by DBHDD to be certified as a Developmental Disability Professional (DDP); Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis. 

Essential Duties and Responsibilities:

  • The Developmental Disability Professional will ensure that all services are delivered utilizing a Person-Centered Focus.
  • The Developmental Disability Professional (DDP) will have a specified schedule with sufficient hours per week to meet the duties of the DDP and level of need for individuals receiving services, which includes but are not limited to:
  • Oversee the services and supports provided to individuals that include:
    • Monitors and/or participates in the implementation and delivery of the Individual Service Plan (ISP).
    • Supervises the delivery of service and ensures that strategies reflect the goals and objectives of the ISP.
    • Monitors the progress toward achievement of goals in the ISP, and makes recommendations for modification to the ISP, as appropriate.
  • Supervise the formulation of the individual’s plan for delivery of all waiver services provided to the individual by the provider, on an annual basis subsequent to ISP development and after any ISP addendum that includes, but is not limited to:
    • Ensuring the implementation strategies reflect the ISP and the needs of the individual.
    • The agency DDP participates in the development of the ISP.
  • Conduct functional assessments to support formulation of the individual’s plan for delivery of all waiver services that include:
    • The Health Risk Screening Tool;
    • The Supports Inc. Functional Behavioral Analysis, if qualified;
    • And others as needed or required, if qualified.
  • Supervise high intensity services that address health and safety risks for the individuals that include:
    • Reviewing and/or writing, and the implementation and effectiveness of the Behavior Support Plan.
    • Reviewing and/or writing, and the implementation and effectiveness of the Crisis Plan.
    • Identifying ongoing supports as needed (medical and/or behavioral) in collaboration with appropriate personnel.
  • Performance Indicators of the responsibilities listed above are as follows:
    • Active participation in the planning meeting documented in either the meeting minutes/notes and/or progress notes prior to ISP meeting.
    • Documented contact with the SC prior to the ISP date.
    • Consulted with, supervised, and provided guidance to direct support staff regarding implementation of the services.
    • The DDP will complete documentation in any individual’s record for any of the above responsibilities. This documentation should include the signature, title/credentials, timed (start and end time of delivery of service) and date.
    • The DDP will complete, or assure the completion of required agency assessments, included, but not limited to, HRST and SIS, within the given time frame.
  • All Developmental Disabilities Professionals must be trained in all required trainings identified in the Community Service Standards for All DD Providers.
  • Complete and renew all required Trainings for All Developmental Disabilities Professionals
  • Other required trainings for DDPs in their first year of employment include:
    • Individual Service Planning
    • Supports Intensity Scale overview
    • Health Risk Screening Tool on line training.
  • The provider agency must also document the participation of each DDP in a minimum of 8 hours per year of additional DBHDD sponsored or other training in the area of developmental disabilities, not listed above or included in the Community Standards for All Providers.
  • Complete case file reviews for the Villager’s Goal Tracking Sheets, Trip Summary, Daily Case Notes, current Individual Support Plans (ISP), psychological and social information, participation and attendance notes, skill functioning assessments, legal forms and documents for employment.
  • Compiles monthly documentation and reports for tracking and attendance records submitted to the Social Services and Finance Departments by the specified due date. Monitors Villager accounts and Comings and Goings.
  • Schedule and facilitate the pre-Individual Support Plan (ISP) meeting for each Villager on caseload – as per procedure to better acclimate the Villager to the climate of the official ISP meeting.
  • Assists Support Coordinators with scheduling and facilitating Individual Support Plan Team meetings, represent the Wavier Program and advocate for the Villager on subjects pertaining to goals.
  • Train Day Program Skill Instructors and Skill Instructors Aides on the hopes, dreams, and visions of the Villagers as specified in each approved Individual Support Plan.
  • Supervises the day program staff including hiring, disciplinary actions, and annual evaluations- and counseling as needed.
  • Communicates with each department, the specific services and supports identified in the Individual Support Plan (ISP) for implementation of appropriate services for each Villager receiving supports and services. 
  • Secures information such as medical, psychological and social factors contributing to Villagers situation and evaluates these and Villager's capacities.
  • Counsels Villagers individually, in family or in small groups regarding plans for meeting needs.
  • Helps Villagers to modify attitudes and patterns of behavior through team approach and decision by increasing understanding of self, personal problems and Villager's part in creating them.
  • Responsible for developing the Quality Assurance project for the Community Access program.
  • Schedules and facilitates Quarterly Interest Survey Meetings for the Villagers receiving Community Access Group Services at the Day Program.
  • Creates quarterly calendar of activities including cost breakdown according to the Person Centered needs of the Villagers utilizing the Villager Interest Survey results.
  • Facilitates support team communication and meetings; including scheduling meetings, taking and distributing memos and correspondence, and following up on concerns expressed by team members.
  • Provides annual evaluations for department employees within the prescribed time frame.
  • Coaches and counsels employees as needed to ensure job performance.
  • Interviews and hires staff to fill vacancies in a timely manner.
  • Performs disciplinary actions up to and including termination of employees as needed.
  • Attends all required and assigned trainings and meetings.
  • Participates in social events and activities as needed, such as parties, outings, etc.
  • Must be trained to assist Villagers with medications as needed.
  • Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.

The Quality of Performance will be evaluated at least annually using the Annandale Village performance evaluation process.

Employee is required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
