Position Description: Coordinate and implement the Community Access Program. Oversee and ensure the implementation of supports and services identified in each Villager’s Individualized Support Plan. Serve as an intermediary for the Villagers, families, other agencies, and support coordinators, by collecting data, goal tracking information, progress notes, schedules, events and activity documentation. Serves as the Developmental Disability Professional (DDP) for the program by following all regulatory requirements for the position.
Minimum Qualifications:
Bachelor's Degree in Social Work or related human services field and/or 5 years of experience in a human services setting, preferably in MR/MH/DD. Must be able to meet the minimum requirements outlined by DBHDD to be certified as a Developmental Disability Professional (DDP); Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis.
Essential Duties and Responsibilities:
The Quality of Performance will be evaluated at least annually using the Annandale Village performance evaluation process.
