We want you to grow your career with us!About the Position:Primary function is to monitor the quality improvement program and provide ongoing education. Reports directly to the Compliance Officer.Essential Job Functions:Provides ongoing clinical record review to evaluate documentation to determine the quality and appropriateness of the care provided, including services provided under arrangement.Reviews all types of OASIS assessments to ensure appropriateness, completeness and compliance with federal and state regulations, organizational policies and procedures and HIPAA guidelines trends.Ensures agency clinical records contain appropriate ICD-10 coding and sequencing as it relates to the patient’s medical condition and comorbidities.Review agency medical records for effective interchange and coordination of care documented in the record.Reviews clinical record for documentation of mandatory client notifications at appropriate time points.Reviews medication profile for completeness and accuracy.Reviews consent for treatment for completeness, accuracy and safeguarding patient rights.Identifies documentation trends and implements appropriate action planning, documentation, and follow-up.Reviews visit utilization for appropriateness of care guidelines and patient condition.Prepare Quarterly Improvement Report per agency provider number including evaluation of CASPER reports.Notifies organization leadership of problematic trends as a result of OASIS review.Works with managers to address trends that affect the agency’s outcomes and process measures.Ensures clinical records are maintained in compliance with state and federal guidelines and organizational policies.Completes clinical review for active and discharged records.Ensures medical necessity criteria, homebound criteria, and F2F criteria is met within the clinical record.Provides education on CASPER report data to clinical leadership as required.Provides education on the QAPI process as required.Meets mandatory continuing education requirements of the licensing board.Carries out other duties as assigned by the Compliance Officer.Required Skills & Abilities: OASIS certification (COS-C) preferred.Home Health ICD-10 coding certification preferred.Knowledge of OASIS, ICD-10 coding.In-depth knowledge of anatomy and physiology and ability to communicate effectively with clinical staff.Detail oriented and able to work with minimal supervision.Must have computer literacy skills and read, write, and comprehend English.Qualifications & Experience:Minimum 3 years home care experience in OASIS review, quality assurance, utilization review, home care coding, quality performance improvement, or clinical record management.Professional clinician, graduate of an accredited college/university. Current State license, Registered Nurse.Current State Driver’s license.Work Environment:Remote position, 0% travel required.M-F, 8:00 a.m. – 5 p.m. and ability to work flexible schedule to accommodate time zones as needed. We are an equal opportunity employer.
