Job description

Anew Behavioral Health is a mental health agency seeking a professional and compassionate Case Manager in our Mt. Vernon, Ohio office. We are a growing agency that offers positive, strength-based services to help clients struggling with mental health issues. Candidate should be accountable for patient care cycle to help enhance the quality of patient management.

Seeking 20 hours (or more) of work per week. Flexible scheduling allows you to choose the hours that best fit your schedule. Pay starts at $19/hour, with higher rates of pay based on direct experience.

Job Type: Contractor

Requirements:

  • Background check required
  • Must pass a drug test
  • CPR Certified preferred
  • Behavioral health experience (1 year preferred)
  • NPI number/Ohio Medicaid ID preferred
  • High School Diploma minimum, College degree in Human Services preferred

Job Duties:

  • Coordinate and provide care that is safe, timely, effective, efficient and client centered.
  • Handle case assignments, draft service plans, review case progress.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Work with therapists, clients and client family members to ensure that client needs are met.
  • Offer facilitation of achieving personal independence and assistance with development of daily living skills. Performs other duties as assigned.
