We are seeking an organized, self motivated 40 hour PTA to be on our Facilities Focused team and skilled home health as needed, assisting these patients in maintaining their maximum level of independence within the facility they reside and provide educational support to facility employees specific to the patient’s needs. This role will require travel to our partnered facilities; that may include, assisted living, residential care living, and/or group homes.
PRIMARY PURPOSE OF POSITION: To provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association at the direction of the Registered Physical Therapist.
EDUCATION: Graduate of an Accredited Associates Degree Program for Physical Therapy Assistants.
EXPERIENCE: A minimum of 1-year recent clinical experience.
