Private Duty Nursing Home Care LPN

Anchor Health Homecare Services is a Brooklyn-based healthcare organization dedicated to providing high-quality and compassionate home healthcare services to individuals in need. We are seeking a skilled and dedicated LPN to join our team. As a private duty nurse with our agency, you will have the flexibility to choose the cases that best fit your schedule and preferences. Each case is assigned on a case-by-case basis, allowing you to accept or reject assignments based on factors such as location and hours of work. Once you accept a case, it becomes your permanent assignment, providing continuity of care for the patient.

Responsibilities:

• Providing skilled nursing care to patients in their homes
• Administering medications and treatments as prescribed
• Monitoring patient vital signs and condition
• Collaborating with other members of the healthcare team to develop and implement individualized care plans
• Providing emotional support and companionship to patients and their families

Requirements:

• Valid LPN license in the state of New York
• Minimum of 1 year of clinical experience preferred
• Homecare experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and prioritize tasks effectively
• Compassionate and empathetic attitude towards patients and their families
