Position: Physical Therapist - Nursing Home

Location: New York, 10075

Requirements:

· Complete a bachelor's degree and doctoral program in physical therapy

· Active Physical Therapy License

· Valid CPR Certification

· Preferably with nursing home physical therapy experience

Responsibilities:

· Conduct thorough assessments of patients, considering their medical history, and mobility limitations.

· Identify the patient's goals and develop individualized treatment plans.

· Develop and implement comprehensive treatment plans based on the patient's needs and goals.

· Provide hands-on physical therapy interventions, including therapeutic exercises, manual therapy, gait training, and the use of assistive devices.

· Monitor and document patient progress and adjust treatment plans as necessary.

· Teach patients how to use assistive devices and mobility aids effectively.

· Collaborate with other healthcare professionals involved in the patient's care

· Maintain open and clear communication with patients to address concerns, answer questions, and provide support.

· Maintain accurate and detailed patient records, including progress notes, treatment plans, and any changes in the patient's condition.

· Communicate patient progress to the healthcare team and the patient's primary care physician.

· Adhere to all healthcare regulations and privacy laws (e.g., HIPAA)

· Uphold ethical standards and maintain professionalism in delivering care.
