PREFERRED QUALIFICATIONS -RN Care Manager for discharge planning and care coordination, requires prior acute care discharge planning experience

Facility Location
Sanford is located in the central region of Florida and used to be home to various Indian tribes who inhabited the shores of Lake Monroe. Orange Groves and agriculture dominated Sanford’s economy until the early 1940’s when it was later deemed cheaper to cultivate in southern Florida as it was a frost-free zone. Today Sanford offers such attractions as the Central Florida Zoo and Botanical Gardens and is also the place where naval aviators go for their operational training.
