StartDate: 6/3/2024 Available Shifts: 12 E Pay Rate: $2404.95 - $2650.35
* Case manager position that will be responsible for care coordination for members on Advanced Medical Care at Home Program (AMCAH). Duties involve enrolling members, conducting virtual visits alongside MDs, medication coordination with Pharmacy, lab ordering, discharging of members, referrals to any specialty programs and Post hospital Appointments
Required Qualifications
Remote/computer work IN a KP Office in Downey
Case Manager experience in Acute Setting
Must have knowledge of Microsoft Excel, Microsoft office and digital platforms.
