AMI Network is currently partnered with a profoundly impactful community-based health care organization that has been around for decades. They are looking for a compassionate Bilingual (Spanish) Nurse Case Manager to join their growing team.

Westminster is a private, non-profit community clinic offering a medical home to the uninsured in Ventura County - 99.8% of the patients coming to Westminster are Spanish-only speakers and youll serve as a primary liaison between them and the providers.

As their Nurse Case Manager, youll touch every patient that comes through the doors during their evening clinics (Tues-Thurs) helping them to understand their medications, interpret lab results, and coordinate all follow-up appointments working closely with their multidisciplinary team.

This is a full time, permanent position offering an annual salary of $80,000-$95,000/yr (based on experience) with benefits provided.

The clinic is open for patients Tue/Wed/Thur from 4p-10p. Aside from those clinic hours, youre free to work from home ! Talk about flexibility, plus no on-call hours!

Minimum Qualifications

• Fluent Spanish Speaker is a must
• CA Nursing license (RN or LVN)
• Clean CA drivers license- you need to give clients a ride to or from an appointment or transport supplies in clinic provided vehicle occasionally.
