AMG Home Healthcare Services is currently seeking an experienced RN for a Lead Case Manager (MEDICARE) position opening. This is for a full-time position at our Home Healthcare office located in Delray Beach, FL.

Case Manager is responsible for directing, supervising, and evaluating the activities of the clinical staff and overall clinical operations for the Home Health agency.

RN Case Manager candidates must have prior experience as a Home Health Case Manager/ADON/DON specifically dealing with Medicare patients. Case Manager will be required to do home health visits, including but not limited to: New patient admissions/OASIS SOC/RECERT/DISCHARGE, supervisory visits, surprise leadership visits, and routine visits when necessary. Experience in dealing with Medicare home health patients is required. Please see position details below:

DUTIES AND RESPONSIBILITIES:

Duties of the Clinical Manager / Director of Nursing Services include, but are not limited to:

1. Responsible for oversight of all patient care services provided by the Agency, whether directly or under arrangement, to meet patient care
2. Oversees, Coordinates and Supervises patient care
3. Defines and prepares job classifications and job descriptions of nursing service personnel and qualifications within the nursing program and ensure orientation of all new staff is carried out prior to first case assignment
4. Evaluating the performance of other individuals in the division in relation to established standards and the individual ' s professional
5. Evaluating total nursing services in relation to Agency
6. Participates in periodic meetings with the administrator to ensure optimum communication interdepartmentally and to discuss any mutual or Agency problem. Participate in all agency in-services
7. Assist in reviewing all logs, including but not limited to Infection Control, Patient Complaints, Incident Reports, on at least a quarterly basis, investigates problems/ potential problems noted; offers recommendations for resolution/change.
8. Lead RN Case Manger/DON is available at all times during operating business hours either onsite or by telecommunication
