AMFM Healthcare is seeking a Clinical Case Manager to join our team as we set new standards in diagnostic and treatment protocols with a compassionate, client-focused approach. Job Type:
Full Time, Salary, Exempt.

Salary Ranges:

  • Clinical Case Manager I - $65,500-$68,000.
  • Clinical Case Manager II - $68,000-$78,000.
Schedule: Monday - Friday, possibility of working Saturday as well as needed.

Job Duties and Responsibilities
include the following. Other duties may be assigned.

  • Proactively develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions. Identify and prioritize individual needs and problems.
  • Be responsible for the client's aftercare plan and make decisions based on the best interest of the client.
  • Lead and provide group therapy sessions utilizing Evidence Based Practices.
  • Case load of up to 12 clients, with minimum of weekly aftercare planning sessions.
  • Coordinate and manage client transfers to higher or lower levels of care and discharges against clinical advice.
  • Communicate with aftercare programs and business development, facilitating aftercare coordination and coordination of care between programs.
  • Initiate and develop comprehensive transition and discharge plans and summaries for assigned caseload.
  • Prepare clinical documentation accurately and within agency time frames.
  • Intervene during crisis intervention situations.
  • Supervise and participate in the agency’s Quality Management system, Orientation, staff meetings, and in-service training.
  • Provide accurate assessment and referral when appropriate.
  • Provide appropriate treatment interventions relative to the specific needs of each client.
  • Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.


Qualifications and Skills Required:

To perform this job successfully, an individual must be able to perform each essential job function assigned satisfactorily.   • Education Requirements: Masters Degree in a relevant field highly preferred. Bachelor's degree minimum.
  • Preferred Licenses and Certifications: Active registration with BBS as an AMFT, ACSW, APCC (or fully licensed) or have active and current CADC I or II.
  • Previous Case Management/Clinical Experience preferred.
  • Experience working with a population that has substance use disorder and/or a mental health diagnoses.
  • Able to pass a federal DOJ LiveScan.
  • Possess valid driver’s license.
