St. John’s United has a mission to provide living opportunities within nurturing environments of hope, dignity, and love. Chapel Court Apartments is an independent living and affordable housing project for older adults ages 62 and older. In order to maintain the cost of living expenses, these residents do not have allocated social services or life enrichment staff to support their daily needs. As people age, the risk for chronic health conditions such as heart disease, type 2 diabetes, dementia, and arthritis increase. In the most recent Healthy by Design Survey, 15% of Yellowstone County adults believe that their overall health is “fair” or “poor” and a lower rating is more often reported among adults age 65 and older. Older adults with lower incomes are also more likely to have disabilities and die younger. DPHHS identifies the five social determinants of health (SDOH) as: economic stability, social and community context, education access and quality, and health care access and quality. SDOH have a major impact on our health, well-being, and quality of life. Increasing access to health care and support networks is a primary strategy to improving health conditions of those experiencing poverty. People with access to adequate support to address health issues are able to lead healthier, longer and more productive lives. Additionally, the cost to society is lower with less strain on healthcare and social systems. This project will alleviate poverty by creating a program that will provide access to coordinated care that includes medical, social, mental, and other services to assist residents into a health standing that will support movement away from poverty.

Member Duties : Help independent living residents at St. John's United improve community connections so they can lead healthy and productive lives! The overall goal of this project is to develop a Resource Directory and accessible network of supportive services for low-income older adults who reside at or receive services from St. John’s United. The VISTA will: complete a Social Determinants of Health (SDOH) survey of residents of St. John’s low income housing project, Chapel Court, to identify community need; create a sustainable Volunteer Plan with a calendar to help increase the mentor base to strengthen and expand resident access to Community Resources; research available providers and services that serve the identified needs to create a Resource Guide; create a Marketing Plan to inform the necessary target audience about the Resource Guide; and build relationships with community resource agencies and health providers to improve services available to residents.
