Home Health Physical Therapist Job Details:
Pay Rate: $1,900-$ 2,025 gross weekly
Schedule: 8:00AM – 5:00PM
Available Shift: Days, Monday-Friday (5) shifts per week

Requirements: Qualified applicants MUST have at least 1 year of experience as a Physical Therapist, have a valid PT license and be willing to obtain MD licensure.
As a Home Health PT, you'll have the opportunity to provide comprehensive care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. As their in-home clinician, you will conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with their interdisciplinary team.
In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum. This mobile role offers independence and differentiation in your day-to-day work, while also keeping you connected to the coordination of care with other healthcare providers like physicians, therapists, and social workers.
