American Home Finding Association is Hiring a Care Coordinator Near Ottumwa, IA

Maternal, Child/Adolescent Health (MCAH) Care Coordinator offers clients assistance in locating appropriate resources, scheduling appointments, and assisting in arranging support services. Must have excellent computer and communication skills. Applicants must have a high school diploma and be ready to work in a fast paced team environment. Other duties include (but not limited to): Answering questions about health care coverage, assisting with establishing medical and dental homes, working with private providers on behalf of families, follow-up to assure clients received services, complete monthly Informing Lists in accordance to Iowa HHS requirements, complete insurance applications for clients, complete data entry in used databases. Applicants must have the ability to use, navigate, and cross-reference a variety of databases. May have responsibility for scheduling MCAH/WIC appointments and taking calls at MCAH/WIC office(s), limited traveling to Agency MCAH/WIC clinic sites, knowledge of community resources, responsible for chart quality assurance checks, filing, and proper handling of client charts and other duties as assigned by supervisors.

Job Type: Full-time

Pay: $15.00 - $16.50 per hour
