```Duties```
- Provide comprehensive home management services to clients, ensuring their well-being and comfort.
- Conduct patient assessments to determine individual needs and develop personalized care plans.
- Coordinate and oversee the delivery of medical services, including medication management, vital sign monitoring, and wound care.
- Collaborate with healthcare professionals to ensure proper utilization management and adherence to HIPAA regulations.
- Assist with discharge planning and coordination of post-hospital care.
- Conduct ongoing case management to monitor client progress and adjust care plans as needed.
- Maintain accurate and up-to-date documentation of client interactions and care provided.

```Skills```
- Strong knowledge of medical terminology, physiology, and patient assessment techniques.
- Experience in a hospital or healthcare setting, preferably as a nurse or healthcare professional.
- Proficient in utilization management principles and practices.
- Familiarity with HIPAA regulations and ability to maintain strict confidentiality.
- Excellent communication skills, both verbal and written, to effectively interact with clients, healthcare providers, and other stakeholders.
- Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
- Detail-oriented with a focus on accuracy in documentation and care delivery.
- Ability to work independently and make sound decisions in a fast-paced environment.
