• DOT # (IF OVER 10K AND NO VEHICLE OVER 26K)
• VEHICLE REGISTRATION

• REAR VEHICLE PHOTO
• PROOF OF INSURANCE
• WORKERS’ COMP OR OCC/ACC – PER STATE STATUTE OR $1MM OCC/ACC
• AMERICAN EXPEDITING LISTED ON COI AS ADDITIONALLY INSURED or WHILE UNDER DISPATCH INSURANCE OFFERED
