$75.00 - $110.00 per hour

Responsibilities:

• The Home Health Registered Nurse (RN) will conduct a detailed assessment of the patient & family during the initial start of care.
• Develop a plan of care with the collected start of care data.
• Case manages patient care episodes and works one on one with patient/ family to improve health and stability.
• Complete clinical notes, orders, and charting in a timely manner.
• Report and consult with physicians and care team regarding patient care and goals.

Qualifications for the Registered Nurse (RN) include but are not limited to:

• Current California RN License
• Current CPR/ BLS Certification
• Valid Driver's License
• 1 Year: RN experience required.
• 1 Years: RN Home Health experience preferred.
