Job Description:
The physical therapy assistant assists in the implementation of the rehabilitative care plan under the direction and supervision of a qualified physical therapist.
Qualifications:
· Graduate of an approved physical therapy assistant curriculum.
· Possesses a current license to practice as a physical therapy assistant in Ohio.
Responsibilities:
· Under the supervision of the physical therapist, assists in evaluation and development of the rehabilitative care plan and in periodic re-evaluation as indicated.
· As directed by the physical therapist, performs routine strengthening and therapeutic exercises.
· Assists in teaching exercise programs to the client and family.
· Consults with the physical therapist regarding the outcome of home visits and reports physical findings in written form.
· Assists clients in use of self-help devices.
· Attends care conferences and Agency meetings as necessary.
· Documents daily progress notes on clients and submits them weekly.
· Communicates effectively to all those providing client care.
Organizational Relationship:
· Reports directly to the supervising physical therapist.

· Must have a reliable car, car insurance and a valid driver’s license.
