Job Description Job Description Amazing Care Home Health Services, Inc. is hiring a qualified RN Case Mgr for its Private Duty Nursing DepartmentCase Management is a collaborave process which assesses, plans, implements, coordinates, monitors, andevaluates opons and services to meet a paent's health needs through communicaons and available resourcesto promote cost-effecve quality outcomes.Descripon:Provide the outcomes-oriented care and oversight to a given paent caseloads in collaboraon with otherregistered nurses, licensed vocaonal nurses, home health aides, therapists, physicians and other team members.Responsible for the overall coordinaon of services provided to paents by the other licensed and under licensedpersonnel.JOB DUTIES/ KNOWLEDGE:Works closely with scheduler: Field staff on scheduling, to ensure that all paents are assigned andscheduled with appropriate disciplines and frequencies.Responsible for receiving all incoming calls from skilled nurses, relaves, physicians, paents and otherdisciplines.Responsible and accountable for following through with all incoming calls and problems through adequatedocumentaon with the use of inter-office communicaon form/occurrence variance reportfrom when applicable.Work closely with scheduler to ensure that all paents are scheduled according to set frequencies: HCAsupervisory visits are assigned and performed q 14 days.Maintain and receive all interoffice communicaons reports, telephone orders, ensure orders areacvated and 'copy should be kept in a CM folder''.Ensures that Re-cerficaon/Follow-Up visits are scheduled 5 days prior to next cerficaon period.Updates physician on paent progress or any changes in paent's condion.Receives paent referrals and responsible for assigning RN to perform admission in absence of an intakenurse. Confers with third party payers as needed.Receives verbal reports on all new admissions, post hospitalizaon assessment und re-cerficaon followup from RNs on the day of service provided.Reviews all admissions, re-cerficaon/ follow-ups, post-hospitalizaon packets prior to forwarding toData Entry Dept.Performs daily paent update, verbal reports from primary nurse & review wrien weekly reports toassign paent for weekend visit if applicable.Performs as paent advocate between the field staff: physician. other disciplines, referral resources, andother involved agencies.Assigns Director of Clinical Services in staff performance valuaon and counseling (verbal or wrien) asapplicable.Assist Director of Clinical Services in staff performance valuaon and counseling (verbal or wrien) asapplicable.Funcons in the Office as a resource person to all field staff.Facilitates team conference to update field staff an any changes related to paent care or problems,review agency policies and procedures and provided staff with mini in-service resources related to fieldstaff specialty to include monthly home health aides in services.Rotates on-call as assigned.
