Assurance Support Servicesstrives to assist persons with disabilities in reaching the highest level of independence possible while on life’s journey. This position is designed to improve or maintain a member’s physical, emotional, and economic independence in the community with or without supports.

ILST Responsibilities:

·Provide daily assistance/training/coaching in light housekeeping and other in-home activities

·Assist the client(s) with budgeting, financial management, money handling, and consumer skills

·Prompt/coach the client(s) to manage medical appointments, medical supplies and prescriptions, clothing, seasonal needs, and shopping

·Provide transportation navigation training

·Assist with recreation and community access

·Establishes a schedule for attendants

·Train the client(s) to advocate for oneself to ensure needs and wants are expressed

·Prevent and/or make known issues of abuse, neglect, or exploitation

·Assist the client(s) with integrating into the community

·Collaborate with human services case managers as needed

·Set up Meal Delivery and Transportation

·Revise LST Care Plan when needed

·Maintain confidential personnel files

Qualifications:

• CNA/CMA/CHHA: 1 year (Preferred)
• Associates degree preferred but not required, depending on work experience
• Must have reliable transportation and a valid license

Pay Rate: $20.00/hour
